Start with the change you want
Is the priority discomfort, an implant problem, or a different appearance? Separate the clinical concern from the shape you want.
Your remaining tissue, skin and previous surgery affect the result. Smaller breasts, loose skin or unevenness may remain; an early photograph cannot predict the settled shape. [1]

A lift reshapes loose skin and repositions tissue. Examination helps determine whether to combine it with removal or stage the procedures. [2]
Where does fat grafting fit?
Fat grafting may soften hollows if donor fat and tissues allow. It cannot reliably replace all implant volume or tighten loose skin. Retention varies. Risks include fat necrosis, cysts, calcifications and infection. [3]
The capsule is a separate decision

A soft, unremarkable capsule may stay. Symptoms, imaging or implant findings may support partial or total removal, weighed against the risks of additional dissection. [4]
Total capsulectomy means removing the entire capsule; it need not come out in one piece. En bloc has a specific cancer-related meaning: removal with a margin of uninvolved tissue for suspected or established implant-associated cancer after appropriate investigation. It is not a routine upgrade for every removal. [5]
When assessment comes before appearance

New swelling, a lump, increasing firmness, pain or shape changes need assessment. Silicone rupture may be silent; imaging may be indicated. Symptoms alone cannot identify the cause. [6]
Seek prompt medical care for fever with redness or rapidly worsening symptoms. Chest pain or shortness of breath requires urgent local care, not a wait for an overseas appointment.
Removal is still surgery. Bleeding, infection, fluid collection, altered sensation, scarring and further surgery are possible. A larger operation is not automatically a better choice. [7]

Does everyone need the whole capsule removed?
No. Capsule treatment depends on the clinical findings and surgical risks. En bloc removal is distinct from routine total capsulectomy and has a specific cancer-related indication.
Can I have removal and a lift together?
Sometimes. Suitability depends on your tissues, health and the required operation. A staged approach may be more appropriate in some cases.
